“Doctor, my tablets are not working anymore. Is there an injection that can settle this once and for all?” I hear some version of this almost every week in my clinic, usually from someone who has been fighting rheumatoid arthritis or lupus for years and is exhausted. Rituximab is often the medicine we discuss next. It is powerful, it works differently from the usual pills, and it comes with a fair amount of fear attached to it, most of it from half-read WhatsApp forwards.
This guide is meant to clear that fog. If you are considering rituximab treatment in India, whether for rheumatoid arthritis, lupus, or a related autoimmune condition, here is what you actually need to know before the first drip goes in.
What is rituximab and how does it work?
Rituximab is a biologic medicine, which simply means it is made from living cells rather than chemically synthesised like an ordinary tablet. It is a monoclonal antibody, a lab-made protein designed to lock onto one specific target in your body.
That target is a marker called CD20, which sits on the surface of B cells. B cells are a type of white blood cell. In healthy people they help fight infections. In autoimmune diseases like rheumatoid arthritis (a condition where the immune system attacks the joints) and lupus (where it can attack many organs including skin, kidneys, and blood), these B cells go rogue and produce antibodies that harm your own tissues.
Rituximab temporarily removes most of these B cells from circulation. Think of it as pressing a reset button on one troublesome part of the immune system. Over the following months your body slowly makes new B cells, and the hope is that the new batch behaves better.
It is given as an intravenous infusion, a slow drip into a vein, not as a tablet or a quick injection at home.
Who is rituximab actually for?
In rheumatology, we do not start with rituximab. It is usually a considered next step, not a first move.
For rheumatoid arthritis, it is typically offered when methotrexate and other disease-modifying drugs have not controlled the disease well enough, or when TNF inhibitors have failed or caused problems. It works best in patients who are seropositive, meaning their blood tests for rheumatoid factor or anti-CCP antibodies are positive.
For lupus, the picture is more nuanced. Rituximab is not officially approved for lupus in the way it is for rheumatoid arthritis, but rheumatologists worldwide, including in India, use it off-label with good reason in certain situations. These include severe lupus that is not responding to standard treatment, lupus affecting the kidneys (lupus nephritis), and lupus causing low platelets or serious blood problems. Using a drug off-label is common and legitimate in medicine, and your doctor should explain why it is being chosen for you.
Rituximab is also used for several other conditions I treat, including certain forms of vasculitis (inflammation of blood vessels), and some blood and immunodeficiency disorders. The principle stays the same across all of them.
How is rituximab given, and what does a session feel like?
Rituximab treatment in India is almost always done as a day-care or short-stay procedure in a hospital or infusion centre, never at home.
A common schedule for rheumatoid arthritis is two infusions given about two weeks apart, and this cycle may be repeated after around six months depending on how you respond. For lupus and other conditions the schedule can differ, so follow the plan your own doctor sets rather than what a friend or online article says.
Before the drip starts, you will usually be given a few medicines to reduce reactions. These typically include paracetamol, an antihistamine like pheniramine, and a steroid such as methylprednisolone straight into the vein. This premedication is standard and important, so do not skip it.
The infusion itself is slow. The very first one can take four to six hours because the drip is started at a low rate and increased gradually while the nurses watch you closely. Later infusions are often quicker. During the drip you may feel a scratchy throat, mild fever, chills, or itching. Tell the nurse immediately if you feel anything unusual. Most reactions settle by slowing the drip or giving extra medicine, and they do not mean you have to stop treatment forever.
Bring something to keep you occupied. A charged phone, a book, a light snack for afterwards, and a family member for company all help. The chair time is long and boring, which honestly is the best kind of boring.
What does rituximab cost in India?
This is the question that decides everything for many families, so let me be straight about it.
The good news is that India has several rituximab biosimilars, which has brought the price down dramatically compared to the original branded product. A biosimilar is a highly similar version of a biologic medicine made after the original patent expires, tested to work in an equivalent way. India was actually one of the first countries in the world to launch a rituximab biosimilar.
As a rough guide, a single 500 mg vial of a biosimilar may cost somewhere in the region of a few thousand rupees to around fifteen thousand rupees, while the original innovator brand can cost considerably more per vial. A full treatment cycle usually needs a total dose based on your body size or a fixed dose depending on the condition, so most patients require more than one vial per infusion.
When you plan your budget, remember the drug cost is only part of the bill. You also pay for the day-care bed, nursing, the premedication, and the pre-treatment blood tests and scans. Add these up before you start so there are no shocks.
A few practical money points for Indian patients:
- Ask specifically whether a biosimilar is being used, and whether a cheaper equivalent brand is available. The difference can be substantial.
- Check if your condition qualifies under any government scheme such as Ayushman Bharat or a state health scheme, and whether your treating hospital is empanelled.
- Many private health insurance policies cover biologic infusions when done as day care, but you must confirm this in writing and understand your waiting periods and sub-limits.
- Keep every prescription and receipt. Reimbursement in India runs on paperwork.
What are the side effects of rituximab?
Every effective medicine carries risk, and honesty here matters more than reassurance.
The most common rituximab side effects are infusion reactions, which I described above. These usually happen during or shortly after the drip and are mostly mild and manageable.
The more serious concern is infection risk. Because rituximab lowers your B cells, your immune defences are reduced for months. This makes ordinary infections more likely and occasionally more severe. In our Indian setting, this is not a small matter. We must actively screen for and think about tuberculosis, hepatitis B, and hepatitis C before starting, because rituximab can wake up a silent hepatitis B infection with dangerous consequences. Your doctor will almost certainly order blood tests for these, and this is not optional box-ticking.
Other side effects worth knowing:
- Low levels of certain antibodies (immunoglobulins) over time, especially with repeated cycles, which can leave you prone to repeated infections.
- A drop in blood counts, which is why periodic blood tests continue after treatment.
- Rarely, reactivation of viruses in the brain, an extremely uncommon but serious problem your doctor is aware of and watching for.
I tell my patients to treat any fever, persistent cough, breathlessness, burning urine, or new skin infection in the months after rituximab as something to report quickly, not to wait out at home.
How rituximab and India’s climate and daily life interact
This is where general international advice falls short for us.
During the monsoon and the humid months, skin and chest infections spread easily. If you have recently had rituximab, be a little more careful than usual. Keep cuts clean, dry your feet properly, avoid wading through flood water, and do not ignore a fever that lasts more than a day or two.
Food and water hygiene deserve extra attention while your immunity is lowered. Freshly cooked hot food, safe drinking water, and washed fruit are simple protections that genuinely reduce your risk of gut infections during this period.
Vaccination timing matters too. Live vaccines are generally avoided around rituximab, and other vaccines work less well when your B cells are depleted. Ideally, discuss and complete recommended vaccines, including the hepatitis B and pneumococcal vaccines where appropriate, before starting treatment. Never take any vaccine without checking with your rheumatologist first once rituximab is on board.
Is rituximab safe in pregnancy?
If you are a woman planning a family, raise this early, before treatment, not after. Rituximab is generally avoided during pregnancy, and effective contraception is usually advised during and for a period after treatment. B cell depletion can affect the baby if the drug is given during pregnancy. Lupus and rheumatoid arthritis both often affect women in their reproductive years, so this conversation is important and completely normal to have. Your doctor can help you plan the timing of treatment and pregnancy safely.
Questions to ask your doctor before your first infusion
Walk into that consultation prepared. Good questions to ask include:
- Why rituximab for me specifically, and what are the alternatives?
- Have my hepatitis B, hepatitis C, and tuberculosis screening tests been done?
- Are we using a biosimilar, and what is the total expected cost for the full cycle?
- What vaccines should I take before starting?
- What warning signs should make me call you or go to a hospital?
- How will we know if it is working, and when?
Frequently asked questions
How long does rituximab take to work?
It is not an overnight medicine. Many patients begin to notice improvement over several weeks to a few months after the infusions. Do not judge it in the first fortnight. Your doctor will track your symptoms and blood markers to decide if it is helping.
Is the rituximab biosimilar in India as good as the original?
Biosimilars approved in India go through testing to show they work in a comparable way to the original biologic. They are widely used across the country and have made this treatment affordable for far more patients. Discuss the specific brand with your doctor, who can advise based on their own experience.
Can I stop other medicines once I start rituximab?
Usually no, at least not immediately. Rituximab is often given alongside methotrexate or a low steroid dose, and stopping your other medicines on your own can trigger a flare. Any changes should be made by your rheumatologist.
Does rituximab increase cancer risk?
